To receive your rebate, you must be 21 or older at time of purchase and legal resident. Please fill out form and upload a photo of your ORIGINAL cash register receipt with STORE NAME, ADDRESS AND PHONE NUMBER printed on the receipt and the required Butternut wine purchase circled. No copied or altered receipts allowed. Incomplete submissions, copied or altered receipts and receipts with no phone number and or no address will be VOIDED.
